Abstract

The TALIsMAn project aims at researching and defining new models and technologies to support vulnerable individuals, voluntary / associationism and operators of social and health services (doctors, nurses, social and health structures). The initiative intends to define the new integrated assistance models able to enable the management of fragility.

The central element in the definition of these models is represented by the new fragility index, which, based on objective performance indicators, will allow to monitor the effectiveness, in terms of impact on the subject's quality of life and the efficiency of the models themselves. terms of use of available resources and of economic sustainability. define, design and prototype an integrated technological infrastructure able to implement, govern, monitor and continuously evaluate integrated care models. The infrastructure will be based on an information model inspired by the principles of Population Health Management (PHM) and composed of the following elements: a framework, based on artificial intelligence, for Big Data Cognitive Environment for classification, correlation, comparison, representation, interactive research data, in order to create an application dashboard for social and health care facilities and operators; a framework for cooperation and interfaces (APIs aimed at interoperability between different smart devices, even wearables, for home care (ADI), a framework for telemedicine services, telemonitoring, remote assistance, telecopulation, telerehabilitation to create sanitary corners also according to the quantified self paradigm, a framework for secure digital data storage based on blockchain, a platform to support the community called HealthLand, which is interactive and enables social networking in order to improve social inclusion.
